4/6/2023 EnchantaVaughn Approved 1.Behavior: QMHP-C greeted Client when QMHP-C arrived to the home. He appeared to be relaxed and ready for the session. QMHP-C engaged his mother in a discussion surrounding his behaviors since the last session. She reported that Client is really struggling with his anger as he will not take his medication when he is mad and he will throw items at his siblings and grandmother when he cannot have his way. 2. Behavior: Client appeared be frustrated at times throughout the session as he would ball his arms up when his behaviors were being discussed about managing self-control. 1.Intervention: QMHP-C engaged Client in role-play scenarios to assist Client with identifying anger management skills and problem-solving solutions. QMHP-C provided Client with information on anger and the definition. QMHP-C explained to Client that the purpose of this activity was to support Client with explaining the ramifications of anger. 2. Intervention: QMHP-C engaged Client in respectful communication rather than interacting negatively with others. QMHP-C illustrated how to manage self-controlling behavior when frustrated and feeling upset. QMHP-C modeled effective ways Client could demonstrate communication and assertiveness. and get mad with his mother at times when discussing his behavior. 1.Response: Client appeared to be receptive to QMHP-Csâ interventions and services that were provided to him during the session. During the session it appears that Client struggles with controlling his anger and frustrations evident by talking back and stomping his feet when upset (as described by his mother). 2.Response: Client demonstrated an understanding of anger management skills and problem solving however he struggles with implementing those skills when needed.
